Why do they need to know that?

When filling out an entry form for a competition you may wonder, “why do they need to know that?”  We hope this explanation will clear things up.

US Figure Skating Number:  This is a seven (7)-digit number which identifies you as a member of US Figure Skating.  Without this number,
you cannot compete.  If you are currently signed up for Learn to Skate you are already a member of US Figure Skating and have a number.
You will be sent a card in the mail…keep it!  If you are a club member, you are a member of US Figure Skating, and you have a number.
Please put this number on the form.  Without it, the registrar cannot enter you into the competition.

Not Applicable:  If you are asked a question that does not apply to you or your skater (i.e. “partners’ name”) than write N.A.  That way the
registrar will know that you did not forget to answer the question.

Birth date:  It is important to include this information because large groups are often split by age and some levels have age
restrictions/requirements.

Gender:  This is more important than it may seem.  Many names are not gender specific and some levels have gender
restrictions/requirements.

Email address:  Please include this if you have one.  This is a very convenient way for competition officials to contact you.  Please make sure
the address is legible.  It is not unusual for the space provided to be way too small for your address.  If that is the case, just write it somewhere
on the form or even on another piece of paper and then just stick it in the envelope along with the form.

Address information:  This is pretty straightforward.  Just make sure you write legibly.

Home Club:  If you are a member of the Boise Figure Skating Club we are your Home Club.  If you are a member of Learn to Skate, Learn to
Skate is your Home Club.  Mary Irving, at the information desk, can help you with your number if you are taking lessons.  Having a US Figure
Skating number does not automatically mean you are a current member.  Dues are renewable yearly so make sure you know your skater's
current status and affiliation either through the Boise FSC or Learn to Skate.  If they ask…our club number is 2310.

Highest US Figure Skating test passed:  Skaters qualify for events based on their test level.  If your skater has passed a F/S test (F/S
stands for Freeskate), they are asking for the highest test passed to date.  If your skater has passed a MITF test, (MITF stands for Moves in the
Field) they are asking for the highest test passed to date.   If you are unsure about this info, just ask your coach…they will know.  (I’ll bet your
skater knows too!)

Coach Email and Phone:  The registrar may need to contact your coach.  Should this be necessary, having their email address and phone
number will save a lot of time.

Signatures:  There are always many places to sign!
Parent Signature:  Please sign wherever this is required.
Coach Signature:  Please have your coach sign after you have filled out the form. Their job is to verify that you are eligible to enter the event(s)
specified.
Official Signature:  Now you can obtain a signature from a board member.  Their job is to certify that you are a full member in good standing
and are eligible to compete.

Physicians Name and Emergency contact information:  You may leave the rink for only a few minutes but that is when things tend to go
wrong.  Your child may be injured and require emergency medical care.

S.A.S.E.:  This is a self-addressed stamped envelope.  Make sure to include a legal sized envelope if you would like a copy of the schedule.
Include your address, return address, and a stamp.

Refunds:  Most announcements now state that refunds will only be given if the event is not held or you withdraw before the entry deadline.  This
is a US Figure Skating rule.  (3235 or 3212 (c) of the Rulebook)

Registrar:  If you need to contact the registrar, please be courteous.  Do not call after 9:00pm, and give this person ample time to help you.  If
possible, do not wait until the last minute.

Payment:  Please follow payment rules closely.  Pay for your practice ice and your events separately if asked to do so.  (You may have to send
them to different addresses too.)  Please read the announcement carefully…it will explain the payment arrangements, and remember…do not
staple the check to the entry form.

How can you help?

Late Fee:
If you miss the entry deadline, include the late fee along with the registration and event payment.  The local organizing committee
(L.O.C.) is more likely to accept your late entry if this fee is included, but there is no guarantee.  Late entries mean a lot of work for all of the
officials so this fee in not an unreasonable request.

Deadline:  If possible, get your entry form in as early as you can.  If 100 skaters enter a competition and they all wait until the last minute to turn
in their form, you can imagine what the registrars mailbox looks like!  Moreover, why is the deadline so far ahead of the actual competition?
Because this is how much time is required to register all of the skaters for all of the events, and schedule the judges.  Until the L.O.C. knows
how many skaters to expect, they cannot make the final plans for the competition.

Write legibly:  If you want your skater to get credit for his/her accomplishments, if you want his/her name to be spelled correctly in the program;
make sure their information can be read.

We hope this answers your questions.  If you need any further assistance please do not hesitate to ask any one of us.  We would be happy
to help!
